General Information on Healthier SG

1. What is Healthier SG?

Healthier SG is a national strategy to transform Singapore’s healthcare system to focus more on preventive care to help residents stay healthy, detect health issues early and manage them well. Under Healthier SG, residents will be empowered to take steps to improve their health proactively with the support of their trusted family clinic and community partners.

2. How will I know if I am eligible for Healthier SG?

Singapore Citizens (SC) and Permanent Residents (PR) can enrol in Healthier SG. The rollout phases will prioritise those who are more likely to need medical care or suffer from chronic illnesses.

From May 2023, residents aged 40 years and above with chronic illness and who are already seeing a Healthier SG GP for the care of their chronic condition can pre-enrol. Your clinic may contact you for pre-enrolment.

From July 2023, MOH will progressively invite Singapore Citizens and Permanent Residents aged 60 years and above via SMS to enrol with a Healthier SG clinic or polyclinic. SMS invitations will also be sent progressively to those aged 40 to 59 years.

3. Will the care provided by different GPs and polyclinics under Healthier SG be the same? What is the difference between going to GPs and polyclinics?

You will receive the same level of care regardless of whether you choose to see a doctor at a GP clinic or Polyclinic as there are standard care protocols to guide doctors in managing chronic conditions such as diabetes, hypertension and lipid disorders. The protocols will also cover health screenings, medications, lifestyle adjustments, and referrals to specialist and acute care when necessary.

Healthier SG GP clinics, like polyclinics, will be supported by a healthcare team managed by their Primary Care Network.

We recommend that you choose a GP clinic that is convenient for you and whom you have been seeing over the years for better rapport and trust.

4. What are the key benefits of Healthier SG?
Having a long-term, trusted relationship with a family clinic allows your doctor to focus more on preventive care to help you stay healthy, detect health issues early and manage them well. Healthier SG empowers you to take steps to improve your health proactively with the support of your trusted family clinic and community partners, every step of the way. The aim is to help you enjoy:
  • A higher quality of life for as long as possible;
  • Peace of mind for yourself and loved ones; and
  • Ease and convenience in accessing health and community services.
Benefits after enrolment include:
  • One-time free first Health Plan consultation with your chosen clinic.
  • Healthpoints (worth $20) through Healthy 365 app after the first Health Plan consultation.
  • Fully subsidised nationally recommended screenings and vaccinations (for Singapore Citizens only).
  • From early 2024, CHAS cardholders with high medication requirements and bills can opt for Healthier SG Chronic Tier, which provides subsidies of up to 87.5% on whitelisted chronic medications, depending on income level.
  • From early 2024, Singapore Residents under the Chronic Disease Management Programme (CDMP) can use MediSave to pay for treatment without cash co-payment.
5. Do I need to be a CHAS cardholder to enjoy the New Healthier SG Chronic Tier under Healthier SG from 2024?
Can I still use my Pioneer Generation (PG)/ Merdeka Generation (MG) card for GP consult?

Yes, you need to be a CHAS cardholder to enjoy the new Healthier SG Chronic Tier, which will be available from early 2024 onwards. Pioneer Generation (PG)/ Merdeka Generation (MG) cardholders may still use their cards to enjoy PG and MG subsidies for GP consultation, and should consider becoming a CHAS cardholder to fully enjoy the new Healthier SG Chronic Tier. You or your household members can sign up at www.chas.sg.

6. Is there a difference in Healthier SG subsidies for each category of Community Health Assist Scheme (CHAS)/Pioneer Generation (PG)/Merdeka Generation (MG) cards?
With the launch of Healthier SG, residents who enrol with and visit their enrolled GP clinic for preventive and chronic care will receive the following Healthier SG benefits:
  • One-time fully-subsidised first Health Plan consultation with the chosen clinic.
  • Healthpoints (worth $20) through Healthy 365 app after the first Health Plan consultation.
  • Fully subsidised nationally recommended screenings and vaccinations (for Singapore Citizens only)
  • From early 2024, CHAS cardholders with high medication requirements and bills can opt for the Healthier SG Chronic Tier, which provides subsidies of up to 87.5% on whitelisted chronic medications. More details of the subsidy levels for different CHAS cardholders under the Healthier SG Chronic Tier can be found in the table below.
  • From early 2024, Singapore Residents can use MediSave to pay for their CDMP treatment at their enrolled Healthier SG clinic without cash co-payment, up to prevailing withdrawal limits.

Enrolment

7. When can I enrol in Healthier SG? Is it compulsory for Singaporeans to join Healthier SG?

Healthier SG will be rolled out progressively in phases from July 2023 onwards. Although enrolment is not compulsory, we strongly encourage you to enrol in Healthier SG. Having a long-term, trusted relationship with a family clinic allows you to stay healthy, detect health issues early and manage them well, supported by your preferred family clinic.

Besides enjoying enhanced healthcare and subsidies, and holistic team-based care from your family clinic, you will also be able to prevent chronic conditions and complications while experiencing peace of mind for yourself and loved ones.

8. How can I tell if my Healthier SG SMS is not a scam?

The SMS(es) from MOH’s Healthier SG will show the registered sender ID “MOH”, in upper case.

We are aware of calls falsely informing recipients that they are from MOH, requesting for your personal or financial information over the phone. Please always verify the authenticity of instructions before offering any personal information. You can also call the MOH Hotline at 6325 9220 if you have any enquiries.

9. Are there plans for those aged below 40 years old to be included in Healthier SG?
Our population is ageing rapidly with older age groups more susceptible to co-morbidities and chronic illnesses. As such, Healthier SG enrolment will focus on those aged 40 and above to proactively prevent them from falling ill and delay the deterioration of health. We will consider further expansions after the roll-out has stabilised. Pre-enrolment will begin in May 2023 for residents aged 40 and above with chronic illnesses and who are already seeing a Healthier SG GP. This will be followed by registration for residents aged 60 and above from July 2023. In the meantime, all residents should continue to stay active and live healthily. There are a variety of community programmes available, including:
  • People’s Association (PA) activities
  • SportSG ActiveHealth programmes
  • Health Promotion Board’s (HPB) National Steps Challenge and Eat, Drink, Shop Healthy Challenge
10. Is there a step-by-step guide for enrolling in Healthier SG?

Step-by-Step Guide to enrol in Healthier SG

Enrol in Healthier SG via HealthHub

  • Eligible residents will receive an SMS invitation from “MOH” to enrol.
  • Download HealthHub app from Google Play or Apple App Store.
  • Login to HealthHub using Singpass. Click on Healthier SG banner.
  • Select your user profile. Click on ‘Enrol to Healthier SG’ button.
  • Choose Fullerton Health clinic and make an appointment for your first Health Plan consultation.
  • Complete an Onboarding Questionnaire before you see your doctor.

Alternatively, you may approach our Healthier SG Ambassadors at Community Centres (CC) or Joint Testing & Vaccination Centres (JTVC) for assistance.

11. How do I enrol if I do not have a valid mobile phone?
Can my caregiver enrol me on his/her HealthHub app?

You can grant access to your family members or loved ones to assist you with the enrolment by adding them as your caregiver in HealthHub and enabling permission to manage Healthier SG on your behalf. Your caregiver can then add you as a care recipient to view your health records and manage your e-Services on HealthHub.

Follow the steps below to first add a caregiver.

Please ensure that you have enabled “Healthier SG” access in the Caregiver’s Details page (as shown in Step 5).

After this has been done, inform your caregiver to add your Health Profile under Add a Care Recipient.

Once your caregiver has successfully added you as a care recipient, your user profile will be shown on HealthHub for them to assist with your enrolment if you are eligible to enrol in Healthier SG.

Alternatively, you may enrol in Healthier SG through the HealthHub web portal by logging in with your Singpass if you do not wish to download the app or do not have access to a smartphone.

12. How can I access HealthHub and Healthier SG-related functions on behalf of my loved ones?
Can I assist my family members to enrol in Healthier SG?
Can we enrol our care recipients or family members?

Yes, with the consent of your family members, you can be given access to help them enrol in Healthier SG and view their Health Plans on the HealthHub app.

Please follow the steps in the below graphics to set up access as a caregiver on your care recipient’s HealthHub app.

Please ensure that your care recipient have enabled “Healthier SG” access in the Caregiver’s Details page (as shown in Step 5).

Next, you can then add your care recipient (or loved ones) under ‘People under my care’.

After it has been added successfully, you can view your care recipient’s user profile in the Health Profiles page. You may continue to assist your loved ones to enrol in Healthier SG and access Healthier SG related functions on their behalf by following the below steps.

13. Can I enrol with a polyclinic instead of a GP clinic?

The care provided at the polyclinics and GP clinics are both guided by the same care protocols. You are strongly advised to enrol with a GP clinic where you would be more likely to see the same doctor for each visit. By seeing the same doctor each time, you can develop and enjoy a long-term trusted relationship with a doctor who knows you well. Healthcare clusters and community partners will work with your chosen clinic to support your needs. Do continue to follow up with your GP if you have been seeing the GP clinic previously.

14. Will I be allowed to change the GP / polyclinic after I have successfully enrolled with Healthier SG?

Yes, you can switch clinics after you have enrolled.

You can do so via HealthHub or go to a Community Centre or Joint Testing & Vaccination Centres (JTVC). You may also call the MOH hotline at 6325 9220.

We will support one change of enrolled clinic in a year to accommodate personal preferences and changes in life circumstances, e.g., families who move house. As some residents may need more time to find a clinic that they are comfortable with at the start, we will support up to four changes for the initial enrolment period.

15. I have recently changed my mobile/contact number, how do I update it for Healthier SG?

You may call the MOH hotline at 6325 9220 or approach the Healthier SG ambassadors at the nearest Community Club or Joint Testing & Vaccination Centres (JTVC) for assistance to update your contact number.

16. What if my preferred Healthier SG clinic is popular and I am unable to enrol or make my appointment?

You can choose to enrol with other Healthier SG clinics suggested in the HealthHub App, or wait for the next available appointment date.

17. How do I check my Healthier SG enrolment status?

You are considered to have successfully enrolled when you are able to view your selected clinic’s name under “Your Enrolled Clinic” tab in HealthHub.

18. I have been seeing various doctors at GP clinics as well as the polyclinic.
Which doctor should I enrol with, and can I continue to see multiple doctors after the enrolment?

If you are already visiting a regular Healthier SG GP clinic, you are encouraged to stay to develop and enjoy a long-term trusted relationship with a doctor who knows you well.

We encourage you to commit to visiting one family clinic who will support you in developing and adopting a Health Plan. The Healthier SG family clinic will serve as the first point-of-contact to holistically manage your health.

19. How do I change my enrolled Healthier SG clinic on HealthHub?

20. Why can’t I find my Healthier SG clinic of choice?
What do I do if my existing General Practitioner (GP) is not a Healthier SG clinic?

For Healthier SG, residents can enrol only with a clinic that is participating in Healthier SG. GP clinics that are not Healthier SG will not be available for selection.

As you will need to enrol with a Healthier SG GP clinic to access Healthier SG benefits, you are strongly encouraged to choose from the list of Healthier SG clinics in HealthHub.

You may continue to visit non-Healthier SG clinics even after your enrolment. However, Healthier SG benefits will not be applicable at clinics that you are not enrolled with, so you would only be able to enjoy the existing schemes such as CHAS and MediSave.

If your current GP is not on Healthier SG, you could also encourage your GP to onboard Healthier SG.

21. Why do I need to visit the clinic after enrolment?
What can I expect at this first Health Plan consultation session?

During the Health Plan consultation at your enrolled Healthier SG clinic, the doctor will discuss your health, and co-create a Health Plan that works for you. This could include:

  • Discussing ways to improve your health, such as by making lifestyle adjustments;
  • Reviewing what health screenings and vaccinations you need; and
  • Booking your next health check-in.

Your first onboarding health consultation will be free, and completing your Health Plan discussion will earn you bonus Healthpoints (via the Healthy 365 app).

Nationally recommended screenings and vaccinations will be fully subsidised at your enrolled clinic if you are eligible.

The first Health Plan discussion is fully subsidised, and you do not need to pay for the discussion. If you are seeing the doctor for your first Health Plan discussion and for other illnesses, please check with the clinic whether there are charges for consultation/treatment unrelated to Healthier SG.

 

23. Can I use my next appointment at the GP/Polyclinic for my first Health Plan discussion?

If you have enrolled with your existing GP/Polyclinic, you can use your next scheduled appointment as an opportunity to also have your first Health Plan discussion with the doctor. However, please note that there may still be fees charged for consultation and treatment that are unrelated to Healthier SG.

24. If I have a company doctor and/or any other healthcare insurance plans, can I still enrol with a Healthier SG clinic?

Your existing health benefits and insurance coverage (e.g., provided by your employer) can co-exist with Healthier SG. MOH encourages employers and insurers to support their panel of clinics to join Healthier SG, so that their employees/clients can tap on medical benefits from employers/insurers and also be supported by the Healthier SG clinic they enrol with for more personalised and comprehensive care offerings.

25. Do I have the flexibility to choose a preferred clinic near my residence or workplace?

Residents can choose any clinic that is participating in Healthier SG. This can be a clinic located near your home or workplace or a clinic that you have visited for many years.

Selection of your preferred Healthier SG clinic can be done within the HealthHub app as you enrol in Healthier SG.

Through HealthHub, you will first see up to three clinics which you frequently visit but you can also use the search bar which allows you to choose a family clinic of your preference that is not on the displayed list.

Planning my first Health Plan discussion

26. How do I book my first appointment for my Health Plan discussion?

Follow the steps below to book your first consultation with your enrolled Healthier SG clinic for your health plan discussion:

Step 1:
– Access your HealthHub app and log in with your Singpass.
– Click on the Healthier SG banner in green on the homepage or select ‘Healthier SG’ in the Service Categories page.

Step 2:
– Follow the instructions within the app to enrol in Healthier SG.

Step 3:
– After enrolment, you will be prompted to the ‘Book appointment’ screen.
– Alternatively, in the Health Recommendations main page, click on ‘Manage appointment’ to schedule a consultation with your enrolled clinic.

After which, you will be able to book an appointment via:
– an in-app booking system;
– email your contact details to the clinic; or
– call the clinic via their contact number provided.

 

27. If I do not have a smartphone, how can I make an appointment?

If your caregiver has enrolled you via the HealthHub app, he/she will be able to book an appointment for you through the same app.

Alternatively, you may approach the nearest Community Club (CC) or Joint Testing & Vaccination Centre (JTVC) for assistance with the appointment booking. Healthier SG ambassadors at the CC or JTVC will provide you with an appointment card to bring along for your appointment. Subsequently, you will receive a Health Plan Booklet at your Healthier SG clinic to keep track of your appointments and guide you towards achieving your health goals.

28. What do I need to bring along for my first visit?

Do bring along:

  • your NRIC to your Healthier SG clinic for verification purposes; and
  • your Pioneer Generation/Merdeka Generation and/or Community Health Assist Scheme (CHAS) cards to enjoy the enhanced CHAS subsidies under Healthier SG (effective only from early 2024).

You are strongly encouraged to:

  • bring along prior relevant health screenings, test result and immunisation records (local or overseas) to share with your doctor; and
  • inform your family clinic of your current medications, therapy, or any other medical concerns.

29. How do I view and access my Health Plan?

A digital summary of your Health Plan can be viewed and accessed on the HealthHub app after your first consultation.

Follow the steps below to view your Health Plan in HealthHub.

For recommended health programmes offered by community partners, you can use the direct link function within the HealthHub app to access Healthy 365 app conveniently and see what activities are available near you. Alternatively, you may download the Healthy 365 app from your app store.

If you do not have a smartphone to access the HealthHub app, you may request your clinic to print out a copy of your Health Plan for you.

30. How often will my Health Plan be reviewed?

Your family clinic will have an annual check-in with you based on your Health Plan and to discuss any changes or next steps you wish to take.

If you have chronic conditions, you can expect to have more frequent check-ins with your family clinic as guided by the care protocols and based on your doctor’s assessment.

31. If I have changed my enrolled clinic, can my current family clinic view my previous Health Plan?

Yes, your current family clinic will be able to view your past Health Plans co-developed with your previous family clinic.

Referral

32. Can my family clinic refer me for specialist care?
Will referrals made under Healthier SG be subsidised at Specialist Outpatient Centre (SOC) rates, similar to those referred by polyclinics?

Yes, your family clinic can refer you for specialist care if it is clinically appropriate. Your doctor (or the care team) will discuss with you prior to making any referral for specialist care.

Singapore Citizens may be referred during their subsidised visits to GP clinics (e.g., under Community Health Assist Scheme, Screen For Life, Vaccination & Childhood Development Screening Scheme) to public SOC for subsidised care. You will be considered as a subsidised patient and will receive care from a team of doctors/dentists. Patients who wish to choose a specific doctor at the SOC will have to pay private rates.

33. Who will manage my clinical referrals after seeing my family clinic?

Your Healthier SG GP clinic’s care team will facilitate your referrals to specialist outpatient clinics (SOC) and/or Hospital as part of your care (if required). Do inform your clinic’s team if you have a preferred SOC and/or Hospital.

There may be situations where you may be required to provide information or submit documents to complete the referral. Your Healthier SG GP clinic’s care team will advise you if you need to fill up documents as part of the referral process.

34. Who do I contact should I need to amend my appointments for referrals?

Please inform the clinic you were referred to that you would like to change your appointment. If you are unsure of the clinic you were referred to, please contact your enrolled Healthier SG GP clinic for clarification.

35. Will my referral results be automatically sent to my family clinic for review?

Not all your clinical results by other care providers will be automatically sent to your family clinic. As such, it is good practice to always bring your clinical results physically during your appointments.

Maintaining my Relationship with my Family Clinic

36. How often do I need to visit my Healthier SG clinic?

Your clinic will advise you on your next check-in dates.

Those with chronic conditions will typically require two to four follow-up consultations annually with a doctor. Prevailing subsidies, such as the Community Health Assist Scheme (CHAS), will apply for visits and treatment. The enhanced Healthier SG benefits will apply for residents seeking care from their enrolled clinic.

37. Will all my health records and screening/tests results be viewable on HealthHub app?

HealthHub retrieves your health records such as screening and lab test results from the medical records system of various public healthcare institutions, such as polyclinics and public structured hospitals.

You may:

Healthy 365

39. If I do not have a smartphone, mobile number nor any caregiver, how do I redeem my Healthpoints?

You may approach our Healthier SG ambassadors at Community Clubs (CC) or the Joint Testing & Vaccination Centres (JTVC) for assistance.

40. How can I earn Healthpoints?

You can earn Healthpoints by participating in healthy lifestyle programmes and challenges on the Healthy 365 app.

41. What can I redeem my Healthpoints for?

You can redeem your Healthpoints for the following reward types:

  • HPB Credit$
  • HPB eVouchers
  • Charity Donations
  • TransitLink eVouchers

42. Will my Healthpoints expire?

Each Healthpoint is valid for 6 months. Your Healthpoints summary details (including its expiry date) can be found under the “My Wallet” tab in the Rewards section of Healthy 365 app.

Programmes

43. Where can I locate Healthier SG programmes to sign up/participate in?

You will be able to find healthy lifestyle programmes by using the Healthy 365 app:

    • Take healthier actions by getting started with challenges such as the National Steps Challenge™ and Eat, Drink, Shop Healthy Challenge, participating in recommended programmes (e.g. Live Well, Age Well programme) and booking activities including a wide variety of MOVE IT free exercise classes and health workshop in the community.
    • Earn Healthpoints for taking healthier actions and redeem Healthpoints by choosing from a catalogue of lifestyle rewards available.
    • Track your daily physical activity and sleep duration with a compatible wearable and use the meal log tool to monitor your daily calorie intake under “My Daily Progress” section, which provides a view of your health-related lifestyle data at a glance
    • Enjoy access to informative resources and community partners programmes to support your journey to a healthier lifestyle.

 

 

44. How will my attendance at Healthier SG programmes be tracked?

Your attendance will be taken through the Healthy 365 app and/or during the programmes by the programme facilitators.

 

45. Do I need to make bookings for Healthier SG programmes, or can I just walk in?

Selected programmes and activities are open for walk in, but slots are subject to availability. To ensure you get a slot, you are encouraged to make a booking beforehand through Healthy 365 app.

 

46. If I do not have a smartphone, how can I sign up for the programmes and keep track of it?

If you or your loved ones do not have access to a smartphone or the Healthy 365 app, you may approach the nearest Community Club (CC), Active Ageing Centre (AAC) or ActiveSG Sport Centres for assistance with the booking of programmes.
