
HealthTrack SG is a year-long pilot programme conducted jointly by the Health Promotion Board, Google, ConnectedLife and Fullerton Health. It complements clinical care with lifestyle support to help patients manage their hypertension, diabetes mellitus or hyperlipidaemia.
The programme combines clinical information with lifestyle data collected through wearable fitness trackers, into the HealthTrack SG app to give patients a holistic view of their health, highlighting the areas where they excel and where they can make improvements.
How HealthTrack SG supports you

** Rewards are given in the form of Healthpoints via the Healthy 365 app upon meeting the programme’s qualifying requirements and specific milestones. Healthpoints can be redeemed for HPB e-Vouchers, SimplyGo e-Vouchers and other rewards.
Eligibility
- Singapore Citizen or Permanent Resident aged 25 - 75 years old
- Have been diagnosed to have any of the following conditions:
- Hypertension or borderline high blood pressure
- Type 2 Diabetes Mellitus or Pre-Diabetes
- Lipid Disorders (including high cholesterol)
- Comfortable with spoken and written English
- Own a smartphone with iOS 15 or Android 10 and above, compatible with HealthTrack SG App
- Comfortable to wear a health tracker during daily activities and sleep
- Assessed by doctor to be suitable to join programme

FAQs
Healthier SG is a national initiative by the Ministry of Health (MOH) to encourage Singaporeans to take proactive steps to manage their health, prevent the onset of chronic diseases and have strong support to lead healthier lifestyles. Upon enrolling in Healthier SG with your preferred family clinic to provide care for your health needs, your doctor will develop a personalised Healthier SG Health Plan to help improve your health. HealthTrack SG complements Healthier SG by allowing you to monitor your lifestyle and health data to achieve the goals in your Health Plan.
Participants will bear the costs for the consultation, blood tests, and wearable device. Fees for consultations and blood tests will be based on Fullerton Health’s prevailing rates. If you are a Fullerton Health corporate client, please check with your HR department regarding applicable corporate rates.
The Programme requires active participation, which includes:
1. Wearing your health tracker during daily activities and sleep
2. Completing the Report Health Status every 2 weeks
3. Interacting with the HealthTrack SG app regularly to monitor your Wellness Score
If there is no data from your wearable health tracker or Report Health Status for more than 90 days, you will be withdrawn from HealthTrack SG and your account will be terminated. Following programme withdrawal, future visits with your healthcare provider at Fullerton Health will not be part of the programme and you will no longer be eligible for rewards.
You may still participate in HealthTrack SG if you have enrolled in Healthier SG.
